If you change your name, address or telephone number, please let our receptionists know by either:
- completing the online form below
- calling in and completing a change of address form,
- download the form here, or
- use your online services account – please note you can only use this method if you already have an online account.
Name changes
If you have changed your name you will need to bring in proof of this ie marriage certificate or other documentation.
Outside of our practice boundary?
If you move outside the practice area you may need to find a doctor in your new area you can do this by contacting Patient Services on 01484 464000.
Alternatively you can log on to NHS Choices website at www.nhs.uk and click on find GP’s then put in your post code and chose from there.
