Access to medical records

The practice is registered with the Data Protection Act 2018 (DPA 2018). Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.

You are entitled to view or receive a copy without charge; however,  please note that in certain circumstances a charge may be made.

You should also be aware that in certain circumstances your right to see some details in your health records may be limited in your own interest or for other reasons.

Once the data controller has all the required information your request should be fulfilled within one calendar month starting from the date the Practice is in receipt of your request.

(In exceptional circumstances where it is not possible to comply within this period you will be informed of the delay and given a timescale for when your request is likely to be met).

Summary Care Record

If you’re registered with a GP surgery, you’ll have a Summary Care Record unless you’ve chosen not to have one. It contains basic information including your allergies, medicines and any reactions you’ve had to medicine in the past. By storing all this information in one place, it makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.

Follow this link to the NHS website for more information on how to access your health records

GP2GP

GP2GP allows patients’ electronic health records to be transferred directly, securely, and quickly between their old and new practices, when they change GPs.

Your data matters to the NHS

Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.

You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.

Your choice will only apply to the health and care system in England. This does not apply to health or care services accessed in Scotland, Wales or Northern Ireland.

Follow this link to find out how this data is used and how to opt out

Enhanced Data Sharing – Your Options – Your Choice

The General Practice or Clinical Service you are attending uses a secure computerised records system, which allows clinical staff to share information in your record to ensure the best care can be provided to you.

A facility is now available in this system which allows you to have control over how your record is shared between the different teams of clinical staff treating you both now and in the future.

You will be asked at each location where you are treated to agree to make your clinical record shareable with other NHS staff that you are under the care of.

At any time in the future you can request that this is changed

Here are some very useful patient leaflets which will help you to make and informed choice; however, if you feel you would like to speak to us about this please contact the surgery and ask for Mahmood Yaqoob – Practice Development Manager or Rachael Atkinson:

Information for patients about sharing preferences: Final-Information-on-sharing-for-patients-implicit-consent

Patient frequently asked questions and answers: Patient-FAQs

(You can pick up these leaflets from our Practice in reception; just speak to any member of our team if you would like to us to explain in more details and we can then record your sharing preferences).

There is also more information available by clicking on this link: https://www.nhscarerecords.nhs.uk/carerecords

If you want to opt out of sharing your records with other Health Care Professionals invloved in your care (not recommended) please download this form and bring it to the surgery – please ask for Rachael Atkinson – IM & T Data Officer or Justin Wood Reception Supervisor for recording.
