How to register and practice catchment area

We are accepting new patients who live in our practice area.
Dr N Chandra & Partners Practice catchment area includes the following post codes

You can register by completing a registration form which can be downloaded from the link below or obtained from our receptionists at the practice. You will need to bring with you some form of identification such as passport or I.D. card.  Once we have your completed registration forms we will ask you to make an appointment to see either the healthcare assistant or in some cases the practice nurse for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date. Or. If you have difficulty finding a practice please contact Patient Services on 01484 466000 who will allocate you to a practice list in your area.

If you want to join the practice please complete a registration forms in full. Downloaded as below or pick up a form from our receptionist. Please bring the completed form along to reception with your I.D. If you know you NHS number this will speed up the process of getting your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.

The Partners at the North Road suite wish to confirm that all patients are allocated a named GP who is responsible for patients’ overall care at the time of registration and the patient will been informed who their named GP is. (If you have not been informed you  can contact the practice  to find out who is your named accountable GP).

If  you wish to have an alternative preference recorded as to which GP that is, the Practice will make reasonable efforts to accommodate this request.
